Terms of Service
Effective October 11, 2026
These terms govern your use of Gathered In Forms (“the Service”), operated by Brushy Creek Software (“we”, “us”) at gatheredinforms.com. By using the Service, as an event organizer or as someone completing a registration, you agree to these terms and to our Privacy Policy. If you do not agree, do not use the Service.
1. What the Service does
The Service lets organizers create registration pages for stake and ward activities. Registrants complete the information on the Permission and Medical Release Form, sign it electronically, and submit it. Each registration is delivered to the organizer’s own Google Sheet and Google Drive folder. The Service is provided free of charge.
2. Not affiliated with the Church
The Service is an independent tool. It is not owned, operated, sponsored, or endorsed by The Church of Jesus Christ of Latter-day Saints or Intellectual Reserve, Inc. The Permission and Medical Release Form and the “Conduct at Church Activities” guidelines are © Intellectual Reserve, Inc., and are reproduced so that units can use the official form. Organizers are responsible for confirming that the form and process meet current Church guidance for their activity.
3. Organizer accounts
- You sign in with a link sent to your email address. Keep access to that email account secure; anyone who can read it can sign in as you.
- You must be at least 18 years old and acting for a Church unit or with the knowledge of its leaders.
- The information you enter about an event must be accurate. The event organizer name, phone number, and email address you provide are shown publicly on the registration page and printed on each completed permission form.
4. Organizer responsibilities for registration information
Registrations contain personal and medical information, often about minors. When you connect a Google account and open registration, you agree to:
- use registration information only to plan, run, and keep participants safe at the activity;
- keep it confidential and share it only with leaders or medical personnel who need it, as the form promises registrants;
- protect the Google account, spreadsheet, and Drive folder that hold it, and control who they are shared with;
- delete it when it is no longer needed, in line with Church guidance and applicable law; and
- respond to registrants who ask you to correct or remove their information.
Once a registration reaches your Google account, it is in your custody, not ours.
5. Registrants and electronic signatures
- Information you submit must be accurate and complete to the best of your knowledge. Event organizers rely on it to prepare for health concerns and emergencies.
- If the participant is a minor, the registration must be completed or approved by the participant’s parent or legal guardian, and the parent or guardian must sign.
- By drawing or typing your name in a signature box and submitting, you agree that this electronic signature is your signature on the Permission and Medical Release Form and has the same effect as a handwritten signature, and you consent to signing electronically.
- Download a copy of the signed form for your records before you submit. The Service does not keep a copy.
6. Google accounts
Connecting a Google account lets the Service create a spreadsheet and folder in that account and add registrations to them, as described in the Privacy Policy. Your use of Google services is also governed by Google’s terms. You can disconnect at any time.
7. Acceptable use
You may not use the Service to:
- collect information for any purpose other than a genuine Church activity;
- impersonate a Church unit, leader, or any other person;
- submit false registrations, spam, or content that is unlawful, harassing, or harmful;
- interfere with the Service, probe it for vulnerabilities without permission, or attempt to access other organizers’ events or data; or
- violate any law, including laws protecting privacy and children’s information.
We may remove events or suspend access that we reasonably believe breaks these terms or puts people at risk.
8. Not medical or legal advice
The Service records information that registrants provide. It does not review, verify, or evaluate that information, and nothing in it is medical or legal advice. Organizers remain responsible for supervising activities, reviewing registrations, and responding to health and safety needs.
9. Availability and changes
We aim to keep the Service available but do not guarantee that it will be uninterrupted, error-free, or available for any particular event. We may change, suspend, or discontinue features or the Service at any time. Keep your own copies of anything you need; registrations are already stored in your Google account.
10. Disclaimer of warranties
THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE”, WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT, TO THE FULLEST EXTENT PERMITTED BY LAW.
11. Limitation of liability
TO THE FULLEST EXTENT PERMITTED BY LAW, BRUSHY CREEK SOFTWARE WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR LOST DATA, ARISING FROM YOUR USE OF THE SERVICE, AND OUR TOTAL LIABILITY FOR ANY CLAIM RELATING TO THE SERVICE WILL NOT EXCEED ONE HUNDRED U.S. DOLLARS. SOME JURISDICTIONS DO NOT ALLOW THESE LIMITATIONS, SO THEY MAY NOT APPLY TO YOU.
12. Indemnity
If you use the Service as an organizer, you agree to indemnify and hold harmless Brushy Creek Software from claims arising from your events, your handling of registration information, or your breach of these terms, to the extent permitted by law.
13. Ending use
You may stop using the Service at any time. Organizers can delete individual events from their event list, disconnect Google from an event’s Google Drive step, or delete their whole account from Account settings in the account menu. These terms continue to apply to past use, and sections 2, 4, and 8 through 14 survive.
14. Governing law
These terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws rules. Any dispute will be resolved in the state or federal courts located in Williamson County, Texas, unless applicable law requires otherwise.
15. Changes to these terms
We may update these terms. The effective date above shows when they last changed. Continuing to use the Service after a change means you accept the updated terms.
